ASSUMPTION OF RISK AND RELEASE OF LIABILITY
I acknowledge that pole dance and aerial fitness activities involve physical exertion and carry inherent risks of injury, including but not limited to bruises, strains, sprains, falls, and other physical injuries.
ASSUMPTION OF RISK
I understand and acknowledge that:
- Pole fitness and aerial activities are physically demanding and involve risks of injury
- These risks cannot be eliminated regardless of the care taken by the studio, instructors, or myself
- I am voluntarily participating in these activities with full knowledge of the risks involved
- I assume full responsibility for any injuries, damages, or losses I may sustain
RELEASE OF LIABILITY
I hereby release, waive, discharge, and covenant not to sue Rooted in Motion, its owners, instructors, employees, agents, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, whether caused by negligence, breach of contract, or otherwise.
PERSONAL RESPONSIBILITY
I acknowledge that:
- I am responsible for my own safety and well-being during classes
- I will inform instructors of any injuries, limitations, or medical conditions that may affect my participation
- I will not participate if I am under the influence of alcohol or drugs
- I will follow all safety instructions and guidelines provided by instructors
AGREEMENT TO FOLLOW SAFETY INSTRUCTIONS
I agree to:
- Listen carefully to all safety instructions and demonstrations
- Use equipment only as instructed
- Ask questions if I am unsure about any movement or technique
- Stop immediately if I experience pain, dizziness, or discomfort
- Report any hazards or unsafe conditions to instructors immediately
STUDIO RIGHT TO REFUSE
Rooted in Motion reserves the right to refuse participation to any individual if, in the sole judgment of the studio or its instructors, participation would be unsafe or inappropriate due to physical condition, behavior, or other factors.
I have read this waiver, understand its terms, and agree to be bound by them.
